Enrolled Families
Parents who are enrolled in 4Cs programs and services are connected with an individual Case Manager who will confirm eligibility, discuss child care needs and answer any additional questions you may have about our preschool and subsidized child care programs.
During your time enrolled in one of the 4Cs programs, contact your Case Manager to report changes or discuss upcoming changes to ensure such changes impacting your services will be covered.
Changes can be reported anytime by completing a Change Request form. The form can be submitted to the 4Cs office or directly to your assigned Case Manager.
